Incident Report Form

Most days at FROS-HA won't require an incident report. This form simply helps us become aware of situations that may require follow-up, maintenance, or support.

Please complete this form if you experience or observe something such as:

• An injury, accident, or medical emergency.

• Damage to the building, grounds, furnishings, or equipment.

• A safety concern or potential hazard.

• A security concern or unauthorized access.

• A significant conflict or disruptive situation involving participants or practitioners.

• A facility issue that requires prompt attention (water leak, electrical issue, broken lock, etc.).

• Any other event that you believe FROS-HA should be aware of for the safety, care, or stewardship of the sanctuary.

 

When in doubt, it's always appropriate to let us know.
